Position : Woman and Family Support Specialist
Reports to: Housing Program Manager
Job Description:
This position will be responsible for coordinating and delivering services to both the Home Connections and YWCA Emergency Transitional Programs. These responsibilities will include maintaining appropriate client files and data in accordance to our service delivery contracts, providing direct case management services to HIV/AIDS infected participants and families both on and off site, and collaboration with other service providers in the Cambridge /Somerville area.
Responsibilities:
Coordination of the intake process for both the YWCA Emergency Transitional
Program and the Home Connections scattered site apartments including obtaining all required
Participant documentation.
Provide intensive case management services including assessment, reassessment, service
planning, coordination of care, monitoring of care, follow up and discharge planning.
Develop a wide network of resource services available to family members of the Cambridge and Somerville area who are infected or affected by HIV/AIDS. Provide coordination and linkage with existing service programs to maximize the access of services available to the CCA targeted population.
Implement a ‘Housing Readiness Strategy" to prepare new participants for transition from
Transitional housing to permanent housing.
Act as a liaison between the following: Landlords, Property Management Agents, Local Housing Authorities, courts, other AIDS services providers, and local homelessness services providers.
Attend weekly staff meetings, case reviews, supervision and training’s including external
Meetings with other service providers.
Complete all assigned reports ,data,and client records in accordance to the mandated timelines
set by CCAs contracted funding sources.
Qualifications:
Bachelor’s degree in related field or equivalent experience working with people from multi-cultural communities. Must have strong organizational skills, ability to advocate for formerly homeless women and families living with HIV, knowledge of substance abuse, harm reduction, domestic violence, mental health, and housing subsidies. Effective communication verbal and written a must. Candidate must exhibit the ability to work independently and as a team player. BI-Lingual Skills (Haitian Creole or Spanish) preferred.
